When you arrive at Spinal Health Care Center you will need to fill out some initial paperwork. When you are finished Dr. Smith will complete a full consultation followed by a full chiropractic examination to assess your neurological, postural and general wellbeing.
To expedite your visit and help us to provide the best possible care you may download and fill out the necessary "New Patient Form" under the Paperwork tab on this website. If you are unable to print the paperwork in advance, we can provide it to you when you arrive at the office.
